Immigration Medical Examination: Form I-693 / Medical Exam
An immigration medical examination is mandatory for most EB-5 investors. The results are submitted through Form I-693 (Report of Immigration Medical Examination and Vaccination Record).
The examination must be conducted by a USCIS-designated Civil Surgeon in the United States or by a Panel Physician authorized by the U.S. Embassy abroad.
For investors adjusting status inside the United States through Form I-485, Form I-693 must be submitted together with the I-485 application in the sealed envelope provided by the Civil Surgeon. The envelope must not be opened by the applicant. USCIS may reject the filing if the envelope is opened or shows evidence of tampering.
For investors applying through Consular Processing in Vietnam, the medical examination must be completed at a U.S. Embassy-approved Panel Physician facility, usually several weeks before the visa interview.
